Begin to Solve Your Dispute

First, telephone (214) 754-0022

Second, a case manager or staff member will mail or fax to you an intake form. You may save time by downloading and printing the intake form. Click on intake or go to the table at the top of this page. Provide all the requested information and return it to DMS with the required fee.

Third, a case manager will contact the person you designate to determine if he/she is willing to mediate the dispute. Your case will be scheduled for mediation at a time convenient to all parties, and a volunteer mediator will be assigned.

Fourth, if you have a court order to mediate or you receive a letter from DMS concerning a case, call the office at (214) 754-0022 for information and/or assistance.
